The purpose of this study is to investigate the influence of change management strategies on organizational performance of public hospitals in Kenya, a case study of Riruta Level 4 Hospital. Public hospitals in Kenya face numerous challenges, including resource constraints, inefficient service delivery, and increasing demand for quality healthcare. Effective change strategies such as benchmarking, adoption of modern technology, organizational restructuring, and phased implementation of reforms are considered essential for enhancing efficiency, staff performance, and patient satisfaction. The purpose of this study is to investigate the influence of change management strategies on organizational performance of public hospitals in Kenya through a case study of Riruta level 4 hospital. Specifically, the study assessed the influence of benchmarking, evaluated the role of technology adoption, analyzed the effect of organizational restructuring, and examined the impact of phased change implementation on the organizational development of Riruta Level 4 Hospital. The study was underpinned by Lewin’s Change Management Model, Kotter’s 8-Step Change Model, Rogers’ Diffusion of Innovation Theory, and Organizational Development Theory. A descriptive research design was adopted to allow an in-depth examination of the relationship among the variables. The target population comprised 47 medical and non-medical staff. The study adopted a census approach since the target population was small. Quantitative data was analyzed using the Statistical Package for the Social Sciences (SPSS) version 28, employing descriptive statistics to summarize demographic information and perceptions of change management practices. Multiple linear regression analysis was used to examine the relationship between the independent variables (benchmarking, technology adoption, organizational restructuring, and phased change implementation) and the dependent variable (organizational development). Hypotheses were tested at a 0.05 level of significance. The findings revealed that benchmarking practices such as stakeholder involvement, data confidentiality, and monitoring mechanisms positively contributed to organizational development. Technology adoption, including the use of digital systems for patient registration and medical records, also enhanced efficiency, though its effect was relatively low compared to other factors. Organizational restructuring was found to significantly improve development through transparent redeployment, clear communication, and orientation of staff. The regression analysis revealed that all four variables including benchmarking, technology adoption, organizational restructuring, and phased change implementation had a positive and significant influence on organizational development at Riruta Level 4 Hospital, benchmarking (B = 0.031, p = 0.003) and technology adoption (B = 0.020, p = 0.016) were significant but showed relatively weaker contributions, indicating that while they improve efficiency, their overall effect is less compared to other variables. Organizational restructuring (B = 0.376, p = 0.002) had a stronger influence showing that measures such as staff redeployment, proper orientation, and clear communication during transitions meaningfully enhanced hospital performance. The most influential factor was phased change implementation (B = 0.382, p = 0.001), which showed that breaking down projects into manageable phases, monitoring progress, and involving staff at each stage strongly drives organizational development. The study concluded that while all four factors positively influenced organizational development, organizational restructuring and phased change implementation had the greatest impact. It recommends that public hospitals strengthen benchmarking and technology adoption, while giving priority to structured restructuring and phased approaches to change for sustainable growth and improved healthcare services.
